Supporting ACC Clients
We are excited to share that ConneXu is enhancing the lives of people living with an injury under the ACC Living My Life contract. This contract includes, but is not limited to, spinal injuries and traumatic brain injuries (TBI).
We are currently supporting people throughout the Bay of Plenty and Waikato/King Country, in their own homes and communities, by breaking down barriers, guiding, coaching and connecting people. We work alongside individuals and their families so people can live their best lives.
How we can support people with an acquired injury to get back the lives they want to live:
Setting goals; your Pathway Map – we help you to develop a visual plan of your aspirations so you can have the satisfaction of ticking off your goals as you achieve them.
Independent Facilitation – if one of your goals is to get back into feeling part of the community, we identify opportunities that suit your unique interests, skills and experience, including cultural, employment, disability and other community events.
Tailored Supports – we support you to have meaningful, active participation in the community based on your unique goals.
Coaching to Self-Manage – we provide guided support to help you manage your own services and supports.
